Healthlincs Care Transformation Organization (CTO) approved by and in partnership with the Maryland Department of Health and Centers of Medicare and Medicaid (CMS) to implement the Maryland Primary Care Program (MDPCP). MDPCP is a value-based care initiative designed to provide patient-centered care to Medicare fee for service (FFS) patients who live in Maryland.
Our CTO provides services that are integral to meeting the care transformation requirements, and our partner providers benefit from the CTO clinical & technical assistance tailored to their needs. We help practices with personnel and expertise to:
